Welcome! To make your first visit smooth, please bring:

  • Photo ID and insurance card (if applicable)
  • Any recent X-rays or records from a previous dentist
  • List of current medications
  • Completed new-patient forms (we can email these ahead)

We'll do a full exam, discuss your goals, and create a plan that works for you. Expect your first visit to take about an hour.
